Aortic Dissection: A Critical Diagnosis

One of the most striking cases presented was an abdominal CT scan revealing an aortic dissection. Dr. Lasker emphasized the importance of thorough examination, even when the primary complaint might not immediately suggest vascular issues:

"At first glance you think ABDO pelvis and if you don't have the vast vessels as your check area you may walk away thinking yeah out of the pelvis normal no abnormalities no perforation ischemia etc etc but that just being able to just sort of have a look at the altar or even doing the thing where it's also doing the thing where you actually have a look at the aorta to try and decide what phase Imaging that you're at that can also help you."

Key points in identifying aortic dissection:

1. Look for a linear density difference between two areas of the aorta
2. Identify true and false lumens (generally, the more dense area is the true lumen)
3. Check for secondary findings such as ischemia or thrombosis
4. Assess organ perfusion, particularly in the kidneys and bowel

Pneumomediastinum: The Continuous Diaphragm Sign

Another fascinating case involved a chest X-ray showing pneumomediastinum. Dr. Lasker guided us through the diagnostic process:

1. Check lung apices and costophrenic angles
2. Look for linear radiolucency along the mediastinum
3. Identify the continuous diaphragm sign
4. Confirm findings with CT imaging

Dr. Lasker noted, "You can see there's a linear radiolucity that goes all the way along like so like a line that goes all the way along like so right so therefore and then someone said it is a continuous diaphragm sign yet I think so."

Systematic Approach to Chest X-rays

Throughout the webinar, Dr. Lasker emphasized the importance of a systematic approach to reading chest X-rays. He outlined his "check areas":

1. Lung apices
2. Costophrenic angles
3. Behind the heart
4. Mediastinum
5. Under the hemidiaphragms

This methodical approach ensures that subtle abnormalities are not missed, even when the primary complaint might not be chest-related.

Interstitial Lung Disease and Traction Bronchiectasis

The webinar also covered a case of interstitial lung disease with traction bronchiectasis. Dr. Lasker provided a clear explanation of the pathophysiology:

"What's happening is that you've got your Airways which are like this which we've kind of established and they've got kind of very and they kind of thin and thin as you go along but you've got your lung architecture all around it and so what can happen is that you can get so pre-bronchial thicken but you can also get something called traction bronchiectasis."

This explanation helps radiologists differentiate between various types of bronchiectasis and understand the underlying lung architecture changes in interstitial lung disease.

The Value of Clinical Context

Throughout the webinar, Dr. Lasker consistently emphasized the importance of clinical context.
