Dr. Imran Lasker's Monthly Case Webinar #22: From Calcified Lymph Nodes to Diverticulitis

The case revealed calcified lymph nodes in the mediastinum, leading to a discussion on differential diagnoses such as sarcoidosis, tuberculosis, and histoplasmosis.

"When you look at that right heart border generally speaking when you look at the next x-ray or any other chest x-ray you should be able to see that right heart border really easily very very distinctly but when you can't it must mean that it's a very specific part of the anatomy it's going to be the right middle lobe," Dr. Lasker explained.

This case highlighted the importance of recognizing subtle signs on chest X-rays and understanding the anatomical relationships in the thorax.

Case 2: MCL Injury Evaluation (12:10)

The webinar then moved on to a knee MRI case, showcasing the value of multiplanar imaging in assessing ligamentous injuries. Dr. Lasker guided the audience through the evaluation of the medial collateral ligament (MCL), demonstrating how to differentiate between deep and superficial fiber injuries.

"You've got deep fibers and superficial fibers of the MCL and sometimes you get a tear of the Like A disruption of the deep fibers sometimes you get a tear of the superficial fibers," Dr. Lasker noted.

This case emphasized the importance of understanding the mechanism of injury and correlating it with imaging findings to arrive at an accurate diagnosis.

Case 3: Abdominal Pain and Diverticulitis (43:38)

A CT abdomen case presented a patient with abdominal pain, leading to a discussion on the systematic approach to abdominal imaging. Dr. Lasker demonstrated how to evaluate for diverticulitis and its complications, including the identification of localized perforation and the absence of fecal material spillage.

"This looks like a diverticulosis important negative would be is it perforated or if there's a collection so I think we can definitely say there is a perforation," Dr. Lasker observed.

This case highlighted the critical role of CT in assessing acute abdominal conditions and guiding patient management.

Case 4: Head Trauma and the Importance of Multiplanar Reconstruction (46:16)

The final case discussed involved a head CT following a road traffic collision. Dr. Lasker emphasized the importance of using multiplanar reconstructions and maximum intensity projections (MIP) to detect subtle fractures and intracranial hemorrhages.

"I would definitely 100% use those and I I genuinely use them every single time I look at any trauma scans," Dr. Lasker advised, referring to MIP reconstructions.

This case demonstrated the value of advanced post-processing techniques in trauma imaging and the need for a thorough evaluation of all brain compartments.
